On the morning of February 19th at 9:50 am, an ambulance roared out of the Xuxinzhuang Community Health Service Center in Songzhuang Town, Tongzhou District, carrying the relay of life from the community to the large hospital within half an hour. 10 minutes later, the ambulance successfully arrived at the emergency department of Tongzhou Branch of Beijing Friendship Hospital, 7 kilometers away. The whole process from discovering the problem to transferring to the higher-level hospital takes less than half an hour. This life relay across urban and rural areas not only validates the practical effectiveness of hierarchical diagnosis and treatment, but also reveals the tip of the iceberg of medical collaboration in urban sub centers - when experts from tertiary hospitals are stationed in community wards, the smooth referral of medical consortia from top to bottom promotes the improvement of grassroots medical services in sub centers. Originally, at 9:30 am that day, the second floor ward of Xu Xinzhuang Community Health Service Center underwent a routine ward round, led by Vice Dean Liu Bing. At this moment, Xiao Gang (pseudonym) on bed 5 waved his hand to Liu Bing and said, "Doctor, I have severe chest pain, which is difficult to hold back and feel uncomfortable." Combining the patient's age of 58, medical history, symptoms, and more than 20 years of emergency work experience, Liu Bing quickly made a judgment that the patient may have had a heart attack. Quickly perform an electrocardiogram to confirm the diagnosis; Call 120 emergency number to prepare for transportation; Notify the Emergency Department of Tongzhou Branch of Friendship Hospital to prepare for receiving patients... A series of arrangements are made quickly and orderly. Just after 10 o'clock, the patient was sent to the emergency department of Tongzhou Branch of Beijing Friendship Hospital for further treatment. The diagnosis was quickly confirmed, and the patient did indeed experience a sudden heart attack. The smooth referral earned him a valuable opportunity for rescue. The dual identity of a doctor empowers grassroots diagnosis and treatment with resources, thanks to Liu Bing's other identity as the chief physician of the emergency department at Beijing Friendship Hospital. Appointed by the hospital, starting from April 2024, Liu Bing, as the backbone of the department, will be stationed at the Xuxinzhuang Community Health Service Center as the vice president, responsible for ward management. These two institutions have formed a close medical alliance for nearly 4 years. Every year, more than 1000 outpatient and inpatient patients are referred from both sides, including critically ill patients like Xiao Gang. We came with the resources of Friendship Hospital ourselves. Once grassroots patients have a need, we can directly find doctors who can solve problems point by point and person by person. Xu Xinzhuang Community Health Service Center has 20 wards, which is also the second ward of general medicine in Tongzhou Campus of Beijing Friendship Hospital. The two sides maintain close linkage: once the hospitalized patients of Friendship Hospital have stable conditions but need follow-up treatment, they can be transferred to the community with the doctor's judgment and the consent of their families. On the contrary, if there is an emergency situation in the community hospital, patients can quickly be transferred to Friendship Hospital. Here, the core of Liu Bing's job is to enable patients to transfer and stay. How can community hospitals retain patients? The workflow and philosophy at the grassroots level are quietly undergoing changes. Liu Bing compares herself to a "quality controller" who refines the diagnosis and treatment service process, enhances the ability of primary healthcare services, and thus maintains patient stickiness. "Take diabetes foot care as an example. Previously, it was just a simple change of medicine to control blood sugar, but now we have achieved comprehensive management. We will invite doctors from relevant departments of Friendship Hospital to carry out consultation according to the patient's condition, so that the treatment is more targeted and the whole process of standardized treatment can be achieved." Community hospitals have limited beds, so what should we do if patients can't stay? Liu Bing took the initiative to collaborate with nearby nursing homes and transferred patients who only needed nursing services to nursing homes. He also took the initiative to undertake the medical work of 10 disabled elderly people in the nursing home, regularly providing basic diagnosis and treatment at home, and providing full life cycle services for patients. According to Liu Bing's calculations, before signing the medical consortium in 2021, the annual number of visits to Xuxinzhuang Community Health Service Center was around 100000, but now it has doubled to over 200000. Based on the sub center and the construction of a medical consortium with 10 institutions in Tongzhou, similar to the Xuxinzhuang Community Health Service Center, more and more grassroots medical institutions are deeply tied to large hospitals. According to the Tongzhou District Health Commission, a district based medical consortium has been established with four major hospitals as the core, namely Luhe Hospital, Dongzhimen Hospital Tongzhou Campus, Friendship Hospital Tongzhou Campus, and Peking University People's Hospital Tongzhou Campus, covering 22 regional health service centers in the jurisdiction. In recent years, more and more "friends" have gathered around Friendship Hospital, which has also enabled the high-quality medical resources of Friendship Hospital to "penetrate the grassroots". In addition to the opening of the Tongzhou campus in 2018, the Shunyi campus also officially opened in April 2024. Both campuses are key projects for relieving non capital functions and promoting coordinated development of the Beijing Tianjin Hebei region. You Hong, Executive Vice President of Beijing Friendship Hospital, told Beijing Youth Daily reporters that currently, the hospital has signed 27 comprehensive medical alliance agreement units, including 17 in Xicheng Campus, 10 in Tongzhou Campus, and 3 hospitals in Shunyi Campus who have submitted cooperation applications and are in the process of signing agreements. In the next year, it is expected to sign 5 new overall medical consortium units and renew with 7 units according to demand. Taking Tongzhou Campus as an example, with the help of the medical consortium model, Friendship Hospital is like a lush banyan tree, and its high-quality medical resources continue to take root in the sub center. You Hong stated that the hospital has cooperated with 10 secondary and tertiary hospitals and primary health institutions in Tongzhou District to establish a medical consortium, and with two medical institutions to establish a specialized medical consortium for women, children, and rehabilitation. Both parties can carry out two-way referral, technical support, service connection, talent cultivation, scientific research, and discipline construction, forming a model of "division of labor and cooperation, resource sharing, and complementary advantages". Experts like Liu Bing rushed to the grassroots level one after another, like powerful water pumps, connecting hospitals at all levels and bringing the grassroots medical care of the entire sub center to life.
