The guidelines specify five aspects of construction content, including: providing chronic disease prevention, diagnosis and treatment, health management services, as well as core functions such as referral and information collection and circulation for residents in the jurisdiction; Reasonably divide the health service area and the diagnosis and treatment area, achieve clear functional zoning, clear directional signs, and smooth personnel flow; Equip health professionals such as general practitioners, traditional Chinese medicine physicians, public health doctors, pharmacists, nurses, etc; Equip relevant facilities and equipment according to the needs of chronic disease health management services; Strengthen the application of digital intelligence, promote the intersection and sharing of information and data in diagnosis and treatment, physical examination, basic public health services, and other aspects. Grassroots chronic disease health management mainly undertakes tasks such as consultation and screening, diagnosis and treatment, follow-up and health guidance, including providing health consultation and appointment registration, health self inspection, collecting residents' health information, signing or renewing family doctors, classified and graded health management, referral, follow-up, health guidance, data analysis and other services. To strengthen service quality management, the guidelines require grassroots medical and health institutions to strictly implement the relevant chronic disease health management service guidelines, standardize service processes and content. The leading hospital of the tight medical alliance or the relevant specialized disease departments of higher-level hospitals should strengthen guidance and quality control of grassroots medical and health institutions, and continuously improve the quality of chronic disease health management services.
